Guiding an Improved Dementia Experience (GUIDE) Model Aims to Increase Care Coordination, Support for Caregivers

NAPLES, FL (July 10, 2024) Today, Avow Care Services announced they have been selected by the Centers for Medicare & Medicaid Services (CMS) to participate in a new Medicare alternative payment model designed to support people living with dementia and their caregivers. Under CMS’ Guiding an Improved Dementia Experience (GUIDE) Model, Avow will be one of almost 400 participants building Dementia Care Programs (DCPs) across the country, working to increase care coordination and improve access to services and supports, including respite care, for people living with dementia and their caregivers.

“CMS is excited to partner with Avow under the GUIDE Model,” said CMS Administrator Chiquita Brooks-LaSure. “GUIDE is a new approach to how Medicare will pay for the care of people living with dementia. The GUIDE participants are envisioning new ways to support not only people living with dementia, but also to reduce strain on the people who care for them, so that more Americans can remain in their homes and communities, rather than in institutions.”

Launched on July 1, 2024, the GUIDE Model will test a new payment approach for key supportive services furnished to people living with dementia, including: comprehensive, person-centered assessments and care plans; care coordination; 24/7 access to an interdisciplinary care team member or help line; and certain respite services to support caregivers. People with dementia and their caregivers will have the assistance and support of a Care Navigator to help them access clinical and non-clinical services such as meals and transportation through community-based organizations.

"Our acceptance into the GUIDE Model emphasizes Avow's commitment to excellence in dementia care and our dedication to the communities we serve," said Jaysen Roa, President and CEO of Avow, Inc. "We are eager to implement this model, which aligns with our mission to offer support not only to our patients but also to their caregivers, enhancing both their well-being and quality of life."

Avow participation in the GUIDE Model will help people living with dementia and their caregivers have access to education and support, such as training programs on best practices for caring for a loved one living with dementia. The GUIDE Model also provides respite services for certain people, enabling caregivers to take temporary breaks from their caregiving responsibilities. Respite is being tested under the GUIDE Model to assess its effect on helping caregivers continue to care for their loved ones at home, preventing or delaying the need for facility care.

GUIDE Participants represent a wide range of health care providers, including large academic medical centers, small group practices, community-based organizations, health systems, hospice agencies, and other practices.

This model delivers on a promise in the Biden Administration’s Executive Order on Increasing Access to High-Quality Care and Supporting Caregivers and aligns with the National Plan to Address Alzheimer’s Disease

Rachel is also working with yoga instructor Marjorie to launch a Senior Salsa class and continues partnering with the resident-led Lifestyle Committee to bring new ideas to life. The best ideas often come from residents, said Rachel.  When people share their passions, we can create even more opportunities for connection and fun.Resident Judy Johansen agrees.  Rachels enthusiasm is contagious, said I Only Wish Id Moved Sooner: Jeanne Findlaters Glenview Story

For Jeanne Findlater, moving to The Glenview at Pelican Bay wasnt just a change of address it was a discovery. I wish Id done it years ago, she said. I simply didnt realize what this lifestyle could offer.Jeanne knows a thing or two about recognizing exceptional opportunities. In 1979, she made history as the first woman in the country to lead a television station in a Top 10 market, when ABC appointed her Vice President and General Manager of WXYZ-TV in Detroit. Today, the Michigan Journalism Hall of Fame member, says one of the best decisions shes ever made was choosing The Glenview. I love having my own home and my privacy, Jeanne said. But I also love knowing that everything is taken care of. I dont worry about maintenance, repairs, or the little things anymore. Its incredibly freeing. The Freedom to Enjoy Every DayMost afternoons, Jeanne relaxes with a glass of wine, a mystery novel, and sweeping views of the Gulf before heading to dinner in The Glenviews elegant dining room. When shes ready to be social, all she has to do is open her front door. Its like living in a beautiful boutique hotel, Jeanne said. There are concerts, lectures, fitness classes, bridge, mahjongg, and wonderful people everywhere. It all comes together to create a very rich life.An Unmatched LocationFor Jeanne, The Glenviews location is equally extraordinary. Theres nothing else like it, she said. Were surrounded by the beauty of Pelican Bays protected mangroves, and I can take the complimentary tram to the beach whenever I want, enjoy lunch by the Gulf, then walk to waterside shops, the Artis Naples Performing Arts Center, or the Baker Museum. Everything is right here.A Home Thats Truly Her OwnJeanne also appreciates that residents own their homes, allowing her to create a space thats uniquely hers. I opened up my kitchen, added granite, and created a home thats perfect for entertaining, she said. It feels exactly the way I wanted it to.The Difference is the PeopleAbove all, Jeanne says its the people who make The Glenview exceptional. The staff is extraordinary, she said. Theyre caring, thoughtful, and always looking out for us. You feel supported every single day.Looking back, Jeanne has only one regret. I waited too long, she said. Life here is everything I hoped retirement could be.
