When you or a loved one are recovering from a hospitalization or are being discharged from a rehabilitation center such as a skilled nursing facility (SNF), you may need short-term transitional home care. Our nurse-led care and team-focused approach are designed to optimize your health and well-being.

Even if your loved one is receiving skilled nursing care from another agency (such as a Medicare-approved group), the companion care and personal care services we offer through BrightStar® Home Care South Minneapolis Metro provide a wonderful supplement. These services can help meet basic needs such as light housekeeping, companionship, family respite, dressing, personal hygiene and ambulation (walking and moving around).

Professionally delivered care can help reduce undesirable outcomes such as re-hospitalization, medication errors and falls; it may even reduce long-term healthcare costs by helping to prevent the health conditions from getting worse.

Short-Term Transitional Home Care Services

A Registered Nurse (RN) from BrightStar Home Care South Minneapolis Metro oversees the plan of care to help provide a safe transition back home. Our team provides a wide range of services to support a variety of health needs such as:

COPD (Chronic Obstructive Pulmonary Disease): We’ll coach proper inhaler use, provide medication reminders and support habits to improve wellness.

Diabetes: We will conduct foot inspections and perform blood glucose level checks, as well as provide family education.

Heart Attack/AMI (Acute Myocardial Infarction): We’ll provide education and support to develop new lifestyle habits that promote heart health.

Heart Failure: We’ll help manage medication adherence, diet and exercise.

Pneumonia: We’ll provide RN-led medication management, help manage oxygen therapy and offer personal care.

Post-Operative: We will assist with surgical recovery in the comfort of home.

Stroke: We will help with activities of daily living (ADLs) like eating, dressing and mobility, as well as promote fall safety, offer medication reminders and provide transportation to physician appointments or therapy (physical, occupational and/or speech).

Therapy for Post-Op or Stroke: We also have physical, occupational and speech therapists who can provide rehabilitation in your home.

Care Paths Deliver Superior Outcomes

One of the unique services we offer is Care Paths for Falls and Change of Condition. Although care paths are fairly common, we’ve taken this approach farther by building person-centered and condition-specific Care Paths based on your loved one’s unique needs, current conditions and past medical history.

Using our proprietary technology, our caregivers are able to help predict and prevent a fall or worsening of your family member's current condition. When a change of condition is identified, we will communicate that information immediately to the supervising nurse at BrightStar Home Care South Minneapolis Metro and/or your loved one’s physician so any needed action can be taken.

If you or your loved one would prefer to receive transitional care at home instead of receiving short-term, facility-based care, please call us at 952.300.3698 or contact us online. We look forward to sharing how we can help.  Or visit us at - https://www.brightstarcare.com/locations/south-minneapolis-metro/

FAQ: Are There Senior Living Communities That Have All Forms of Care in One Place?

An excerpt from Scott Millers presentation, Florida Retirement Guide: Senior Living, Care Options, Family Choices & Future TrendsFrequently Asked Question: Are There Senior Living Communities in Florida That Have All Forms of Care in One Place?Yes, there are senior living communities that have all forms of care in one place. These communities exist on a very limited basis with a pricey model. Its called a CCRC. A CCRC is a continuing care retirement community, which means every level of care is available on one campus except the hospital. You have independent living, assisted living, memory care, and you have skilled nursing all in one environment. Those come in a lot of different shapes and sizes. Westminster Point Pleasant in Bradenton, Florida is an example. Westminster is a large not-for-profit that has CCRCs all over the state of Florida. Very well run model if thats something youre considering. Some of the disadvantages to a CCRC: most of them are buy-ins, which means you have to come up with a pretty good sum of money up front to be able to get in. Now Im going to give you a sticker shock. Those buy-ins can be anywhere from $100,000 to $1 million, and more. The good ones are the not-for-profits. Be leery of for-profit CCRCs. Because if they take that money up front, you want to be sure that that money is being well-invested. That money is there to create a profit pool, to maintain that property, to invest in that property, to reinvest in its residents, but many of the for-profit companies dont do it that way. So if youre ever looking for that environment, look at the not-for-profit first and then know that youre going to be paying some money up front. Now the advantage. If you went to a Westminster community, for example, it costs you maybe $100,000 to $175,000 up front. The communities are to the nines, theyre beautiful, and their care model is incredible. Whats really nice is the money you put up front can be a declining balance back to you or your estate, and it locks in amongst the people that you would pay for as long as you live. And, its much lower than most assisted living communities. So, you could pay $150,000 up front. Theyll take away 2% of that per month. So over the course of 4 years that money goes away. If you only stay 2, half of it comes back to you. You can pay more and theyll guarantee you a 90% rate back over time. The nice part is the cost of living there which they dont call rent, they call maintenance is somewhere between $3,000 and $4,000 dollars a month, all inclusive. You end up in a place where everything is covered. Try to compare the costs of living at home versus in an assisted living environment. Those figures are important when you understand the total costs that go into that particular community. Nobody really knows how much theyre actually paying at home unless they are very, very budget conscious. But, $3,000 a month to be in that environment is very inexpensive when you consider that its three meals a day, seven days a week, your utilities, all of your maintenance, all of the upkeep, anything you could imagine, and full care delivery as you age. And in these communities you can age in place, so if you have to go over to skilled nursing, you dont pay a dime. If you have to go into memory care, you dont pay a dime more. Florida CCRCs are a great option to consider if you are looking for all your senior care in one place.-Scott Miller, CEO of Florida Senior ConsultingDont wait for a crisis! More Than Bingo!

I began my career in senior living in 1996.  I had just graduated with my bachelors degree in therapeutic recreation.  My first job was working in a nursing home as an activity assistant in Saint Louis Park, Minnesota.  For the next ten years, I devoted my work week to providing seniors with meaningful activities.  It was a joy to bring smiles to people.  The programming that is now offered in senior housing has evolved. While some assisted living communities still offer bingo and bible study there is so much more to look forward to.Social EventsParties and Themed Get TogethersSocial events serve the purpose of bringing people together for the fun of it.  Birthday parties, monthly themed get-togethers and holiday celebrations are other fun ways to bring people together.  Some senior living communities will plan theme parties and design the menu to include a special meal related to the party.  This gives everyone an opportunity to participate.Music is a MustLive music is one activity that almost everyone enjoys.  You can sing along, clap, dance or just listen to the tunes.  Music feeds the soul.  It also helps to bring forgotten memories forward for people that have dementia.Happy HourHappy hour is not just for people who enjoy adult libations.  Non-alcoholic options are almost always served too.  Many assisted living homes include a generous snack to go along with the beverages.Community TripsShoppping and OutingsSenior living almost always offers transportation for scheduled community outings.  First off, most people like to go out to eat.  Even when the senior living offers meals three times a day, getting out allows for a change of scenery.Secondly, seniors like to go to the store and shop.  In this case, the bus will take you from home and drop you off at the store.  Sometimes, a staff member is available to assist you with shopping.  Seasonal trips to the theater, casino, and other places around town offer entertainment outside of facility. When I was an activity director, the residents of the senior living wanted to go sledding during the winter.  One resident had never done this before, and it was something she always hoped to do.  We loaded up the van and took the residents to Buck Hill in Burnsville.Ill never forget the smile on Claras face as she rode down the hill.  As I helped her out of the inner tube she said to me, you made my dream come true.  It is a highlight of my career.Intellectual ProgramsLifelong LearningKeeping your mind engaged in stimulating programs is important as a person ages. Senior living homes are now offering a wide range of programs to do just that, keep your brain active.Lifelong learning is one of the cornerstones at Ebenezer communities.  They combine educational programs with a collection of coordinated arts to keep the mind active. The Waters Senior Living offers a program called The Great Courses.  This award-winning program allows you to learn from experts and professors from institutions all around the world.Online OptionsTechnology now offers many ways for seniors to stay engaged in educational programming.  Whether its Solitaire or Soduko, games like these help engage the mind.  Online courses are a benefit today.  We can learn about events and history all over the world in the comforts of our own home.Creative ArtsArt gives everyone the opportunity to explore and tap into their creative selves.  Whether it is a guided painting class, drawing or journaling, we all have an artist within us.It is amazing to watch something beautiful emerge from a blank page.  There are some senior living communities that have designated space such as an art studio or a woodworking shop. At times, these classes are taught by the residents living there. This is a great way to share a talent and give leadership opportunities to the people living there.Dementia Specific ProgrammingWhen it comes to serving people with memory loss, senior living communities create targeted programs that are designed to give purpose and meaning for those living in memory care. For several years, I worked in an assisted living community that served as a primary residence to more than 70 people living with dementia.  What made this community special was the programming and the people who carried out the services daily. Great care was taken in planning activities that would be meaningful to the people living there.Music always played a big role in bringing forward memories.  Faith based programming was also incorporated into each month.  Sensory programs like baking and special meals were also intertwined throughout the month.  Reminiscing about the past was incorporated into sing along programs and trivia. In memory care, it is essential to know the interests of each resident to design a program that gives everyone an opportunity to participate.Celebrate Activity ProfessionalsJanuary 24th, 2024 is National Activity Professionals day.  It is a day to recognize and thank the creative teams that bring so much joy to the people they serve in senior housing and beyond. Take a moment to say Thank You or write a note of encouragement.Planning creative programs month after month is challenging.  The professionals that chose this career do it because they love making other people happy.Thank you, activity professionals for making a difference in the lives of seniors!  Call us for more information at 651-261-5379.   Written by Janelle Meyers, CSA (R), LALD
