The impact you'll make:

  • Creates connections with residents and assists in maintaining their dignity and independence
  • Assists with the daily physical and hygienic care needs of residents, ie: lifting, positioning, and transporting
  • Prepares residents for activity, meals, travel, etc.
  • Advises supervising charge nurse of residents condition and documents observations
  • Maintains records and flow sheets accurately and timely
  • Maintains confidentiality of all resident information and ensures resident privacy
  • Reports any issues or problems that may arise to supervisor
  • Assists families and other visitors as needed
  • Complies with state, federal, and all other applicable health care and safety standards
  • Promotes and exemplifies the Priority Life Care mission and values at all times


Qualifications:

  • CPR and First Aid Certification
  • Prior experience in long-term care, home health, or related service industries preferred

The CNA must be willing to preform CNA duties in long-term care, assisted living or sub-acute rehabilitation.Shift differential offered for evening shift & night shift. Evening shift differential is $2.00 per hour Monday-Friday and $3.00 per hour Saturday & Sunday. We can use childrens abilities to assist the areas that they have most challenges with. Unicef / Parenting for Every ChildSCOPE: To help provide a safe, healthy, and enrichingenvironment for children and young adults served at Special Kids SpecialFamilies Zachs Place childcare and respite program. This position is under thedirect supervision of the Program Director and/or designee. This can be a full-time (40hr/wk) or part-time (20hr/wk) position.ESSENTIAL DUTIES:  Provide direct care, including physical care to theprograms participants. May include caring for medically fragile individuals  Assist in the implementation of established goals andtreatment in the daily programming of the Center as determined in theparticipants individual plan  Follow the policies and procedures for the care ofZachs Place participants, as established by the Center and the StateDepartment of Human Services. This includes safety plans, reportingemergencies, life skills programming and emergency procedures  Maintain necessary paperwork, including medicationlogs, incident reports, check-in guide.
