What makes Heritage Communities different?

We proudly offer programming for residents living with mild cognitive impairment and dementia, adding purpose and meaning to their lives.

Our goal is for residents to be as independent as possible, have a meaningful place in their community, build a positive self-esteem and have opportunities to contribute to their environment.

Residents with Mild Cognitive Impairment, who continue to live in their selected apartments in Assisted Living

Small group size limited to 8 -12 participants

Structured activities that are developed based on the preferences and abilities of the resident

Provides an opportunity for socialization in a safe and error-free environment

The program operates with groups 2-3 times/week and additional one-on-ones

Daily and monthly documentation is captured, and reviewed with your health services team, Executive Director and Memory Support Director.
