Our therapy program is recovery-focused to create a smooth transition home. For individuals returning home, we provide an Outpatient Therapy program to support continuity of care and continued recovery.Summit Rehabilitation and Care Community has a team of care partners caring for individuals living with progressive dementia, such as Alzheimers disease. We provide specialized programming, focusing on individual needs, in a secure dementia care neighborhood to give families peace of mind knowing that their loved one is safe.
