AtThe Estates at Twin Rivers, we seek to provide you with a seamless transition into our skilled care facility.  Moving can be a stressful and difficult time for a loved one and their family, a skilled nursing facility even more so. First, take a deep breath. The staff at The Estates at Twin Rivers has special experience with ensuring smooth transitions for our residents.  After arriving at the Estates at Twin Rivers and getting settled in, you can expect to meet with a representative from each department to learn about your preferences, usually within the first 48 hours. Behind the scenes, these representatives will be working together as your interdisciplinary care team. They are the driving force in developing the plan of care for all of our residents.The Estates at Twin Rivers staff will provide your medication, medical/hygiene supplies, activities, and meals. The only things you will need to bring are several pairs of clothing, assistive devices you use, and any personal items you would like to make your room feel more home-like. If there is ever anything we can do to make your experience better, please dont hesitate to let us know.Our goal is to make your transition into our facility as seamless as possible. We know that the decision to move into our facility is not one that is taken lightly. We will do everything within our power to help new residents get acclimated to their new environment as quickly as possible.
