MissionThe Community Hospice Foundation of Baldwin County is committed to providing support to individuals or families of those who have a loved one affected by terminal illness including offering financial support, education, bereavement and advocacy.How your donation can helpCommunity Hospice Foundation of Baldwin County is a non-profit organization formed to support individuals and families of those individuals affected by a terminal illness. The foundation raises funds through donations and fundraising.Research demonstrates that individuals and families are typically negatively impacted as a result of an illness. Healthcare costs such as deductibles, copayments, transportation and medications are often not covered benefits. Family income can be reduced due to the lack of income from the patient (if previously employed) and/or other family members that must take time away from employment to provide care for the family members, resulting in insufficient income to meet the needs of the household. The foundation provides financial support to individuals and families that apply and are approved by the Community Hospice Foundation of Baldwin County Board.Bereavement is also an integral component, service and covered benefit of hospice. However, in addition to the bereavement activities provided by hospice, memorial services expenses are not covered, but are vitally important to the grieving process. Financial support for such services are provided by the foundation if an application is made and approved.The hospice benefit is underutilized and often those individuals who qualify are not aware of the palliative care benefits that allow patients to live comfortably with their illness. Hospice staff members are available 24 hours a day, 7 days a week, 365 days a year.Hospice care can also be provided in other locations such as assisted living facilities, nursing homes, or other long-term care facility.Our Circle of Care concept surrounds the patient with support, ensuring the highest possible quality of life.Community Hospice provides support to natural helpers, including family, friends, personal physicians and members of religious or social groups.The Circle of Care Teams are dedicated to helping the patient maintain his/her dignity, increasing their quality of life and providing comfort not only for physical symptoms, but also for their emotional and spiritual needs.
