All Care VNA offers a wide range of services to meet the changing needs of our patients. These services allow our patients to return home from the hospital earlier, remain in their homes longer, and helps to prevent re-hospitalizations. All Care works with the patient, family, and physician to provide a custom-tailored home care treatment plan that is individualized to meet each patients specic needs and goals. Services include: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, and Home Health Aide Support.

Palliative Care is comprehensive treatment of the pain, symptoms, and stress of serious illness. Those who struggle with pain and symptoms brought on by serious illness, but who do not qualify for hospice, often benefit from palliative care. All Cares Palliative Care Program is provided by a team of registered nurses and nurse practitioners who work together with our patients and their doctors to provide an extra layer of support, helping them navigate the stresses of the disease process.Hospice offers specialized care and support to patients and their families who are dealing with a life limiting illness. All Care Hospice is dedicated to enhancing quality of life and bringing emotional peace, physical comfort, and spiritual guidance. Hospice care is provided wherever the patient calls home and is available 24 hours a day, seven days a week.
