Each visit includes an evaluation of your condition, progress, and risk factors.Some of our skilled nursing services include: Cardiac care, including CHF management and education Pulmonary care, including COPD management and education Diabetes management and medication education Post-surgical recovery support Pain management and wound care Orthopedic rehabilitation Fall prevention and safety assessments  Therapy ServicesRehabilitation is often key to regaining independence and improving quality of life. Our therapy team offers support for both short-term recovery and long-term conditions. Occupational Therapy: Helping patients regain or adapt daily living skills such as eating, bathing, dressing, cooking, and coping. Speech Therapy: Supporting recovery of communication, swallowing, and cognitive skills after illness, injury, or surgery. Therapy is tailored in-home for real-world needs, often enhancing outcomes for patients with dementia or cognitive challenges. Physical Therapy: Restoring mobility, strength, and function after injury, illness, or surgery to improve independence and activity.  Support from Medical Social WorkersRecovery extends beyond medical treatment. Our medical social workers provide resources, counseling, and planning for patients and families navigating the challenges of illness or injury. Who Is Eligible for Home Health Care?Patients may qualify for home health if: They are under the care of a physician who certifies the need for skilled nursing or therapy. Their condition requires intermittent care or therapy to improve or maintain health. They are considered homebound, meaning they require assistance or supportive devices to leave home, and typically leave only for medical care or short, occasional absences. Private insurance patients may not need to meet homebound requirements.You may be eligible if: You were recently hospitalized or diagnosed with a new condition Your medications have recently changed You have difficulty leaving home due to illness or risk of falls You prefer receiving care in your home  Who Pays for Home Health? Medicare & Medicare Advantage: For Medicare beneficiaries, covered home health services are provided at no cost. Some Medicare Advantage or private insurance plans may require co-pays.
