Formerly Compassionate Care Companions, Mozaic Companions & Homemakers offers personalized support for thriving at home. We pride ourselves on promoting independence and overall wellness for individuals who desire to remain in their home. 

The Mozaic Hospice team consists of specially trained nurses, physicians, social workers, home health aides, clergy, volunteers and bereavement specialists who work closely together to provide comfort with a focus on pain and symptom management. Emotional support is a cornerstone of care for both patients and family members, and spiritual guidance is offered and available through our network of representative from various faiths.

Mozaic care navigators are available to discuss your needs or the needs of a loved one by appointment or phone call. Consultations may be scheduled at your convenience. Service agreements and terms will be reviewed.
