About Full Circle Health

Since 1975, Full Circle Health has offered full service medical, behavioral health, and outreach services for families, infants, children, adolescents, teenagers, adults and seniors and are an active partner in growing a healthy community.

We serve our community by focusing on providing comprehensive health care to individuals and families. Human beings are the population we serve. Gender, race, ethnicity, nationality, religion or sexual orientation do not interfere, in any way, with the care we aim to provide. We support diversity, compassion, and our mission is to serve all. We constantly strive to make healthcare accessible and equitable to the underserved and vulnerable populations in our community.

Full Circle Health also has a diverse team that provides a rich training environment. Everyone is tremendously dedicated and committed to family medicine and pediatrics education. We have a long history of recruiting excellent residents who are committed to rural and under resourced family medicine and pediatrics. This brings together a rich blend of superb residents that become colleagues in training. We have a team of dedicated personnel who enjoy working with residents and fellows while caring for our patients. This provides a tremendous amount of support to residents and fellows as they advance through our three-year program.

Older Adult Care Services:

Services include routine check-ups, preventive care, and treatment of acute and chronic illnesses. We also offer health education and counseling to help our patients manage their health conditions. Specialized services, such as mental health care, substance abuse treatment, and dental care referrals, are also available.

Our care team professionals, including physicians, nurses, and social workers, work collaboratively to provide coordinated care to our older adult patients. We are thorough as we create unique care plans for our patients.

We accept all insurance plans, including Medicaid and Medicare, and offer affordable care options for uninsured patients. Patients can schedule appointments by phone, and same-day appointments are often available. The center also offers telehealth services for remote consultations if that is preferred.

Care Management:

Care management services involve a coordinated approach to healthcare that focuses on optimizing a patients overall well-being. This approach includes services such as:

Care managers work closely with patients, their families, and healthcare providers to ensure that healthcare needs are met effectively and efficiently. This holistic approach to care aims to enhance the patients experience, improve health outcomes, and promote a higher quality of life.
