Community LIFE

Community LIFE is dedicated to helping older adults remain living independently at home. With eight Health and Wellness Centers across the region, we provide coordinated medical care, social services, therapies, and in-home services to our participants. Services also include, transportation, behavior health services, and home delivered meals. For eligible seniors, there is no cost for services.
