Support for Caregivers

  • Caregiver Program: Assistance for family caregivers of older adults, grandchildren raising children, or adults with disabilities. Services include information, counseling, support groups, respite care, and specialized dementia support programs like Dementia Dialogues and Dealing with Dementia.


In-Home & Community-Based Services

  • Alternatives Program: Helps eligible adults (18+, low-income, at risk of nursing home placement) remain at home. Offers homemaking, home health aides, personal emergency systems, equipment, home modifications, and overnight facility stays.

  • Medicaid Aging Waiver: For individuals 65+ needing nursing-home-level care but wishing to stay at home. Services include case management, homemaking, chores, companion care, limited home modifications, and extras like meals or supportive aides.


Insurance Guidance

  • SHIP (Senior Health Insurance Information Program): Education and support on Medicare A & B, supplemental policies, Medicaid, low-income Medicare programs, long-term care insurance, and fraud prevention. Available through local senior centers and case managers.


Meals & Transportation

  • Home-Delivered Meals: Daily nutritious meals delivered to homebound seniors.

  • Congregate Meals: Group lunches at senior centers.

  • Transportation: Nonemergency rides to senior centers, medical appointments, and errandsoptions include private, shared, and paratransit services; accepts Medicaid and various payment forms.


Senior Centers

  • Four active centers in Blanding, Monticello, Bluff, and La Sal offering: 


    Hot meals, social activities, classes, legal visits, and health education (nutrition, flu clinics) .


Long-Term Care Advocacy

  • Ombudsman Services: Advocates for residents' rights and resolves issues in nursing homes, residential care, and adult foster care facilities.


Elder Justice & Abuse Reporting

  • Educational resources on elder abuse, neglect, self-neglect, and financial exploitation.

  • Instructions on recognizing signs and mandatory reporting to Adult Protective Services or law enforcement.
