UPMC Senior Care provides both in-office and at-home consultations and primary care to older adults with complex health care needs. For patients with multiple health issues, our geriatric specialists co-manage care with their primary care provider. Our goal is to help older patients stay independent, optimize self-care ability, and maintain quality of life. Our providers will diagnose, treat, and manage a variety of conditions including impaired memory (with or without behavioral issues), falls and mobility issues, social and safety concerns (unable to drive, living alone, or being homebound), weight loss, declining health, and serious illness requiring palliative care.
