At Continuum Care Hospice of Sarasota, the goal of our team of nurses, social workers, chaplains, aides, Certified Music Therapists, Certified Massage Therapists, volunteers, and Medical Directors, is to enhance the quality of life for patients with life-limiting illnesses and provide their loved ones with the support they need for the duration of the illness. Helping our patients and their families maintain dignity and quality of life is our focus by providing physical, emotional, and spiritual comfort for the patient; in addition to providing tools, information, and support for the patients family. Continuum Care of Sarasota staff combines strong clinical excellence with a holistic approach to end-of-life care by offering specialty programs such as music therapy, virtual reality experience, and equine Therapy; and other modalities such as massage therapy, reiki, clinical aromatherapy, and a gentle presence. Continuum Care staff is committed to ensuring the end-of-life journey is comforting and peaceful for both the patient and their loved ones .In addition, Continuum Care of Sarasota staff is just as committed to assisting family and facility caregivers by providing support and education programs. Staff at are a assisted living and nursing and rehabilitation facilities are many times intimately involved and are our partners in caring for patients at the end of life.
