The communities can be expensive, too, which can make them an unrealistic choice for some people.

Few transitions in life feel as difficult as the moment when one spouse needs to move to a care community while the other remains at home. Perhaps one partner is living with advanced dementia and needs the security of memory care, while the other is still active and independent. Or maybe a stroke or chronic illness has created care needs that can no longer be safely managed at home, despite the spouses best efforts.These situations bring a complicated mix of emotions, including grief over changing roles, worry about separation, and uncertainty about the future. Layered on top of these feelings is a pressing financial concern: How do we approach funding senior care without leaving the spouse who lives at home unable to afford basic living expenses?The good news is that financial protections exist specifically to address this scenario, particularly when Medicaid is involved. Understanding how income and assets are evaluated when only one spouse needs care, and knowing which funding options may be available, can provide both practical direction and much-needed peace of mind during an already stressful time.Medicaid rules that protect the spouse at homeWhen one spouse needs to move to a nursing home or, in some states, to assisted living through a Medicaid waiver program, special rules help protect the financial security of the spouse who remains at home (referred to as staying in the community, meaning their local community as opposed to a senior living community). In Medicaid terminology, this person is called the community spouse, while the person entering care is the institutionalized spouse.What care settings do these protections apply to? Community spouse protections primarily apply when one spouse enters a nursing home and needs Medicaid to help pay for that care. In states that offer Medicaid waiver programs covering assisted living or memory care, some of these protections may also apply, though the rules can differ. These protections generally do not apply to independent living or other residential settings that dont qualify for Medicaid long-term care coverage.The core principle behind these protections is preventing spousal impoverishment, the idea that paying for one spouses care shouldnt leave the other spouse destitute. Without these rules, couples would need to spend nearly all their joint savings before Medicaid would help, potentially leaving the community spouse with almost nothing to live on.Instead, Medicaids spousal impoverishment protections allow the community spouse to keep a protected amount of the couples joint assets and income. Two key terms define these protections: The Community Spouse Resource Allowance (CSRA) is the amount of the couples combined countable assets that the community spouse can retain without affecting the institutionalized spouses Medicaid eligibility. For 2025, federal guidelines set this range between approximately $31,584 (the minimum) and $157,920 (the maximum), though the exact amount varies by state. Some states automatically allow the maximum, while others calculate it based on half of the couples combined countable assets, up to that maximum. The Minimum Monthly Maintenance Needs Allowance (MMMNA) protects the community spouses income. If the community spouses own income falls below a certain threshold (approximately $2,644 to $3,948 per month, depending on the state), they may be entitled to some or all of their institutionalized spouses income to reach that minimum level. This ensures that the community spouse has an adequate monthly income to maintain their household. These protections make a significant practical difference. Instead of spending down to near-poverty levels, a couple can preserve substantial resources for the community spouses ongoing living expenses, home maintenance, and future needs, while still qualifying the institutionalized spouse for Medicaid coverage. These rules are central to funding senior care in a way that safeguards both partners needs.Paying for senior care: Funding and benefit options to exploreBeyond Medicaids spousal protections, several funding sources may help cover care costs when one spouse moves to a community. Most families use a combination of these options over time and understanding them is an important part of funding senior care responsibly and sustainably.Private savings or retirement incomeMany couples initially pay for care using savings, retirement accounts, Social Security benefits, and pensions. When both spouses have their own income streams, families can often dedicate one spouses income toward care costs while the other spouses income covers household expenses at home.This arrangement can work well in the short term, though it may not be sustainable indefinitely if care costs significantly exceed one spouses income. For example, if assisted living costs $5,000 monthly but the spouse needing care receives only $2,500 in Social Security benefits, the couple must cover the $2,500 gap from savings or other income, a strategy that works until their savings deplete to levels where Medicaid eligibility becomes relevant.Veterans benefits (Aid and Attendance)The VAs Aid and Attendance benefit can provide additional monthly income to help pay for care, and it applies in situations where one spouse needs care while the other remains at home. This benefit is available to qualifying wartime Veterans or their surviving spouses who need help with daily activities.Importantly, when one spouse is a Veteran and the other needs care, the Veteran may claim Aid and Attendance benefits on behalf of their spouse. Similarly, if the Veteran needs care and has a spouse at home, the benefit amount may be higher than for a single Veteran, recognizing the need to support both the Veteran and the dependent spouse.This benefit considers household income and medical expenses, so families paying significant care costs may qualify even with moderate retirement income.Long-term care insuranceIf one or both spouses purchased long-term care insurance years ago, this can be a significant help. These policies typically pay a daily or monthly benefit when the insured person needs assistance with activities of daily living, regardless of the care setting, whether in a nursing home, assisted living, memory care, or even at home.When only one spouse has a policy, that insurance covers their care costs, preserving more of the couples joint assets and income for the community spouses needs. Some policies also include inflation protection or shared benefits between spouses, which can extend coverage considerably.MedicaidFor many couples, Medicaid eventually becomes necessary when care costs exhaust other resources. When one spouse applies for Medicaid, eligibility is determined using special rules that account for the couples marital status.Rather than treating each spouses income and assets separately, Medicaid looks at the couples combined resources, then applies the community spouse protections described earlier. This means the community spouse can retain protected amounts of both assets and income, while the institutionalized spouse can qualify for Medicaid even though the household as a whole has resources above the typical individual eligibility limits.The application process requires substantial documentation, financial records, asset statements, proof of income, and medical evaluations, and the rules around asset transfers and look-back periods are complex. People can receive hospice care in different settings, including where they currently live. Many people receive hospice care in their senior living community so they dont need to move. Here, we explain how hospice care works within senior living communities, including the services offered and who pays for them, to help seniors and their adult children understand what to expect.What is hospice care?Hospice care is a type of care provided to people at the end of life. It is an alternative to traditional medical care in the sense that it emphasizes comfort, symptom management, and quality of life instead of curative treatment for illness. In fact, part of accepting hospice care includes agreeing not to pursue treatments. That might sound scary, but remember that hospice is for a point in time where treatments arent effective. Attempts to treat the condition could even be causing considerable discomfort for little benefit.Hospice care usually includes a range of services and support for both the patient and family. Depending on the persons needs, care may be provided by several members of the hospice team, each playing a different role. Hospice care may include the following types of support: Pain and symptom management. Nursing care. Personal care support. Social work services. Spiritual support. Hospice is provided by an interdisciplinary team that can include: Hospice physicians. Nurses. Medical social workers. Home health aides. Volunteers. Chaplains or spiritual counselors. There are two main forms of hospice. The first is inpatient care, which is provided in a hospital or a stand-alone hospice facility. The second is routine home care, which is provided where the individual lives. This second type is what were focusing on today.Can hospice be provided in senior care communities?Hospice can often be provided in senior care communities, which means a resident may be able to stay in a familiar setting rather than having to move elsewhere for end-of-life care. That said, how this works depends on the type of community, the residents needs, state rules, and the policies of the specific community.Hospice in assisted livingHospice is often available in assisted living, but it is not automatic in every case. Assisted living communities generally focus on personal care rather than ongoing skilled medical care, so hospice services are usually brought in through an outside provider.Whether a resident can stay in assisted living while receiving hospice often depends on state regulations and the communitys own policies. In some cases, hospice makes it possible for a resident to remain in assisted living longer than they otherwise could. In others, the resident may still need to move if their needs become too great for the setting.Hospice in nursing homesHospice is commonly provided in nursing homes. Because nursing homes already provide 24/7 nursing oversight and a higher level of medical support, they are often well equipped to care for residents who are also receiving hospice.In this setting, the nursing home staff continues handling the residents day-to-day care, while the hospice team adds comfort-focused services, symptom management, and support for the resident and family.Hospice in memory careHospice can also be provided in memory care. In many cases, this allows a person with dementia to remain in a secure and familiar environment while receiving end-of-life support.How hospice works in memory care depends on the communitys licensing. Some memory care neighborhoods are part of assisted living, while others are part of nursing homes, and stand-alone communities are typically licensed as one or the other. Because of that, the rules and care capacity can vary. Even so, hospice is often an important added layer of support for residents with advanced dementia or other terminal conditions.Hospice in independent livingHospice can usually be provided in independent living because these communities are residential rather than medical. Residents are often free to bring in outside hospice providers just as they would in a private home, but rules around hospice will vary by community.The main challenge is that independent living does not provide hands-on care. Hospice provides important comfort-focused services, but it does not replace full-time caregiving. If a resident in independent living needs significant help with daily activities, the family may also need to arrange additional private-duty care or consider whether another care setting is more appropriate.How hospice and senior living staff work togetherWhen a senior living resident is receiving hospice care, the facility staff continues to provide many of the services it did before, such as: Assistance with daily living tasks, including hygiene needs. Room, board, and activities. Skilled medical care if the facility is a nursing home. Hospice teams provide: Medical, physical, spiritual, and psychosocial support provided through a multidisciplinary care team. Short-term inpatient care when required for crises or caregiver respite. Support and guidance with care goals and end-of-life conversations. Specialized medical equipment like wheelchairs and oxygen. Support for the family at the time of their loved ones passing. Once hospice care has been ordered, the hospice team and senior living staff work together to create a care plan and organize responsibilities. The care plan developed by the hospice provider in consultation with the senior and their family may involve reducing some of the care currently provided by the senior living staff. Who pays for hospice in senior living?Hospice care is fully covered by Medicare. Its also covered by Medicaid in most places and by many private insurance providers. Families often have no additional costs, but they may sometimes be responsible for copays for medication or short-term inpatient respite care. To be eligible for funding, seniors must meet hospice eligibility requirements. The individual must be certified as terminally ill with six months or less to live, forgo curative treatment for their illness, and choose a provider that accepts their payment method (e.g., Medicare). Medicares hospice benefit does not pay for room, board, or care provided by the senior living community.Hospice in senior care focuses on comfort, dignity, and quality of lifeIts easy to assume that hospice care means youre giving up on life. This may be why many people avoid hospice for as long as possible. However, hospice isnt about giving up at all. Its simply about focusing on quality of life, comfort, and dignity at the time when those things matter the most. This shift in focus gives seniors time to connect with their loved ones and focus on what matters to them while feeling more comfortable. Hospice providers take a person-centered approach, ensuring the seniors values, wishes, and faith are respected at every step. Its also important to keep in mind that hospice isnt an irreversible step. Some people decide to stop hospice care and seek curative treatment because their health improves. Others might opt out because they want to try a new treatment. Some may simply change their minds.
