The Centers for Medicare & Medicaid Services (CMS) recognizes that care management is one of the critical components contributing to individuals improved health, as well as reducing expenses. A Chronic Care Management (CCM) program may help avoid the need for more costly services in the future by proactively managing patient health and functional decline, rather than predominantly treating disease and illness.CCM now allows 20 minutes per calendar month of non-face-to-face care coordination, directed by a physician or other qualified healthcare professional, for Medicare beneficiaries with two or more chronic conditions. Examples of chronic conditions include, arthritis, asthma, chronic obstructive pulmonary disease, depression, diabetes, heart failure, hypertension, and osteoporosis, among others.Only one practitioner may be reimbursed by Medicare for the CCM service provided to a beneficiary during a given calendar month; there is no charge or co-pay to beneficiaries. According to the Centers for Medicare & Medicaid Services (CMS), 58 million Medicare beneficiaries will begin to receive new Medicare cards over a 12-month period, starting in April 2018. This is to eliminate your Social Security Number (SSN) from the card in order to fight medical identity theft. These numbers will be replaced with a Medicare Beneficiary Identifier (MBI) made up of up to 11 letters and numbers to be used for billing, eligibility and claim status.In addition, there are several things you need to know about your new Medicare card:Your new card will automatically come to you. You dont need to do anything as long as your address is up to date.Your new card will have a new Medicare Number thats unique to you, instead of your Social Security Number. Recently, the American Heart Association lowered the bar for high blood pressure from 140/90 to 130/80. These guidelines are based on the detection, prevention, management, and treatment of high blood pressure. When a loved one is seriously ill, finding the best care available is your top priority. This is part one in a four-part series dedicated to an in-depth examination of advance directives: who is eligible to make an advance directive, what it is, how to create one, and what to do with the document after it is complete.Who is eligible to make an advance directive?Every competent person has the right to make decisions concerning his or her own healthcare, including the right to choose or refuse medical treatment. When a person becomes unable to make decisions due to a physical or mental change, such as being in a coma or developing dementia (like Alzheimers disease), he or she is considered incapacitated.To make sure that an incapacitated persons decision about healthcare will still be respected, the Florida legislature enacted legislation pertaining to healthcare advance directives. The law recognizes the right of a competent adult:to make an advance directive instructing his or her physician to provide, withhold, or withdraw life-prolonging procedures;to designate another individual to make treatment decisions if the person becomes unable to make his or her own decisions; and/orto indicate the desire to make an anatomical donation after death.Since anyone is eligible to make an advance directive, the next step is to better understand the document in question. Q: I like the convenience of using my debit card, but Im worried about the security. Are there places I should avoid using my debit card?A: Local law enforcement recently sent a fraud alert that warns consumers to be careful using debit cards in certain places. This warning comes after skimmer machines were discovered on gas pumps at several local gas stations. The Lee County Sheriffs office urges people to think twice before using debit cards at the following locations:Online  Hackers and vulnerable websites are two reasons you should avoid using your debit card online. If your card number is stolen and used, the money comes directly out of your checking account and it could take weeks, if not longer, to get it back.Restaurants  When using a debit card at a restaurant, you give up control of your debit card and trust your personal information will be protected. Although most servers are trustworthy, you may not want to risk getting that unscrupulous server who secretly brings a handheld card skimmer to work.Big Ticket Items  Its safer to use a credit card when purchasing expensive items, such as electronics, vacation rentals, entertainment tickets, etc. When you use a credit card, you can dispute a charge after a purchase, whereas with a debit card, the money comes directly out of your checking account.Gas Stations  Fuel pumps are a favorite target for card skimming thieves as mentioned above.  The devices are used to fraudulently collect credit and debit card numbers and they can be difficult to spot.Suspicious-Looking ATMS  Just like at gas stations, skimmers can be attached to ATMs, so its important to take a good look at an ATM before you use it. Aside from catching up with old pals or searching the latest trends, social media can also make you more susceptible to cyber threats.Recently, The Lee County Sheriffs Office Economic Crimes Unit found a pattern of suspicious behavior involving local individuals targeting friends, high school acquaintances and friends of friends via social media in an attempt to commit fraud. By using sites like Facebook or Instagram, these suspects can identify their victims and lure them into a scam. These types of cons involve the scammer contacting their targets, usually claiming they need help getting money to someone via the targets bank account. In exchange, the suspect will offer to pay the victim money for their good deed.Once the transaction happens, the scammer will deposit a fraudulent check via the banks mobile app and immediately begins withdrawing funds leaving the account empty, or worse, in the negative. As a result, the victims are responsible for the lost money and in some cases may even lose banking privileges or lowered credit scores.Fortunately, there are several ways to avoid becoming a victim of this scam:Do not release your bank account, PIN number, or password information to anyone.Do not allow anyone to deposit checks into your account, unless they are your employment provider or banking agent.Do not give anyone access to your bank account or personal funds.Do not assist in cashing someone elses check; there are business for that.If you are ever unsure of a situation, dont act on it.
